New HCFA form 2014 version 02/12 of CMS-1500 for ICD-10

2014 is the year of major changes and accomplishments in health care industry. Considering the future changes in 2014 and Healthcare industry requirements the revision for new HCFA has been approved by NUCC and CMS. The new Revision of  HCFA CMS-1500 is set to be effective from 1<sup>st</sup> April, 2014 for submitting Medicare or Health insurance claim.</div>

Following are the important highlights of changes in New HCFA for 2014.<br />

<li>Scanable reading QR code is introduced that is replacement of the symbol 1500</li>
<li>New HCFA Provide facility to use ICD-10 codes as well ICD-9 codes</li>
<li>New limit for billing diagnosis codes are 12 that was  previously 04 in old HCFA</li>
<li>To indicate role of applicable referring, ordering and supervising providers the two character Qualifier will be use on HCFA or paper claim.</li>

Provider and Healthcare professionals will be able to submit their claim with ICD-10 codes using new HCFA form version 02/12 because the old HCFA form CMS-1500, version, 08/05 support ICD-9 codes only. New HCFA version 02/12 support billing and coding more than four and up to twelve diagnoses on single claim.<br />

The testing phase or transition of new claim form has already begun, Medicare and other payers have started accepting claims filled on new revised CMS-1500 form version 02/12 from 6th January, 2014.All providers and Health care professional can submit their with using old claim form CMS-1500 version 08/05 till 31st March, 2014.</div>

The Asthma Coding in ICD-10 compared to ICD-9 is quite different. The coding of Asthma in ICD-9 was based on two categories of Extrinsic and Intrinsic Asthma. Extrinsic Asthma also known as allergic Asthma is a form of seasonal allergy while Intrinsic Asthma is also known as exercise induced Asthma and Chemical induced Asthma.<br />
<br />
<h2>ICD-10 Asthma Codes 2014 and Documentation Requirements</h2>The coding of Asthma in ICD-10 is a bit different as it will not be necessary to code for the difference between Extrinsic and Intrinsic Asthma. In ICD-10 coding system, Asthma is categorized in section J45 and will require documentation for mild medium and severe forms of the disease. Each severity of the different types of Asthma will also include additional qualifiers that physicians will be required to specify when administering treatment to patients.<br />
<br />
<h3>Asthma ICD-9 to ICD-10 Conversion and Crosswalk</h3>Physicians in ICD-10 will use code J45.20 for patients with mild intermittent Asthma that is of uncomplicated type, J45.21 on the other hand will be used on patients with mild intermittent Asthma with exacerbation. J45.22 will be used for mild intermittent asthma with status asthmaticus.<br />
<br />
The major difference between ICD-10 and ICD-9 lies on the fact that ICD-10 does not have any separate codes for the coding of chronic obstructive asthma as opposed to chronic obstructive bronchitis. ICD-10 code J45.50 will be used to code a severe persistent type of Asthma that is of the uncomplicated type.<br />
<br />
The code will essentially replace 493.00 that was used to code extrinsic asthma unspecified type of Asthma and 493.10 for the intrinsic asthma unspecified. ICD-9 CODE 493.02 that is being used for extrinsic asthma with acute Exacerbation and 493.12 for intrinsic asthma with acute exacerbation will be replaced by J45.51 which will be used for severe persistent asthma with acute exacerbation.<br />
<br />
ICD-10 will rely on J45.52 for severe persistent asthma with status asthmaticus replacing 493.01 for extrinsic asthma with status asthmaticus and 493.11 for intrinsic asthma with status asthmaticus that is being used in ICD-9.ICD-9 currently uses CODE 493.90 for Asthma unspecified type with 493.91 being used for Asthma unspecified type with status asthmaticus and 493.92 for Asthma unspecified type with acute exacerbation. <br />
<br />
ICD-9 code J45.30 is being used for coding Asthma uncomplicated type while J45.990 being used for other forms of asthma with exercise induced bronchospasm with J45.991 being used for other forms of asthma with cough variant asthma.<br />

<h2>Obesity Coding and Reimbursement Guidelines</h2>The health care industry is soon shifting from ICD-9 to ICD-10 essentially meaning a change in the coding process of obesity as a medical condition.<br />

Morbid obesity is a commonly used term used to refer to patients whose body weight is between 50-100% and 100 pounds more than the normal body weight. This type of obesity can result from reduced rates of metabolism which are required for the burning down of fats in the body. Drug induced obesity with certain anti-diabetic drugs such as thiazolidinedione’s, this mainly causes many diabetic patients to become obese with time.<br />

E66.2 will be used for high obesity with Alveolar Hypoventilation which is a condition for extreme levels of obesity mostly known to cause shortness of breath in many patients. This type of obesity becomes worse when one is sleeping and during periods of Apnea, this type of disease is also characterized with the coloration of the skin due to lack of enough supply of oxygen in the body and lethargy.<br />

Obesity also comes in simple forms that can be easily controlled by controlling the amount of calories ingested in any given day. E66.9 will thus be used in this case. This type of obesity does not come with any form of complication and mostly caused by eating habits.  <br />

The 2013 was the final opportunity for Physicians and eligible healthcare professionals to participate in <b>Medicare E-prescription incentive program</b> and to avoid adjustment in Medicare payments. Medicare will apply adjustment in 2014 payment of the Physicians and Eligible healthcare professionals who did not started using Electronic Prescription in 2013.<br />
